Location for HD Support Group”
The Lake Zurich HD Support Group is now the Libertyville HD Support Group
Effective June 14, 2010 and the second Monday of every month,
the Libertyville Support Group will be held at the
following location:
Advocate Condell Medical Center - Conference Center/Day Center
700 Garfield Avenue, Libertyville, IL
New Start Time: 7:00 P.M.

Directions to Advocate Condell Medical Center:
Take Milwaukee Avenue (Rt. 21) to
Golf Road (across from Weil Cadillac).
Turn West on Golf Road (if you are coming from the south, turn left; if you
are
coming from the north, turn right). Take Golf Rd. to Garfield (1st
4-way stop). Turn right on Garfield to the
Emergency Entrance on your
right. Turn into Emergency Entrance, stay to your left and continue to
Conference Center
on your right. There is ample parking available.
