Tribute Gift

Here's your opportunity to make a donation in honor of a friend or relative or someone important to you. When you make a tribute gift, we ask for your name, address and the honoree's name and address.

We'll send a card to the honoree acknowledging your thoughtfulness (you can add your own message too). You'll also receive a card from HDSA – Il. Chapter to confirm your donation and advising you that a gift card has been sent to the honoree. All Tribute gifts will be acknowledged (with your permission) in the HDSA – Il. Chapter Newsletter, Hopes and Dreams.

 

Memorial Gift

A meaningful and loving way to honor the memory of a loved one.

We ask you to fill in your name and address, the deceased person's name, and the name and address of the person to whom you want us to send an acknowledgment (to you or to the person you designate as the recipient).

 

You can add your own message to the card. You'll receive a card from HDSA – Il. Chapter confirming your donation and advising you a gift card has been sent in your name. All Memorial Gifts will be acknowledged (with your permission) in the HDSA – Il. Chapter Newsletter, Hopes and Dreams..

Memorials and Tributes

Memorial envelopes are a tasteful way of honoring a deceased relative or friend and at the same time contribute to the struggle against Huntington's Disease.  We now have memorial envelopes on hand and can make them available for visitations, funerals, and/or memorial services.  We will be happy to supply them to funeral homes for their general use.

There are many ways to be involved and/or support the Illinois Chapter.  Each of us have special talents that we can share.  Perhaps you would like to volunteer to help with an event that has a special meaning for you or to be on a committee.  Whatever your talent or the level of involvement you want, YOU are important to us.  To volunteer or for more information, please contact Susie Hodgson.
